How can I replace my shower head?

The pipe connecting my shower head leaks every time we turn it on.

My question is how can I replace this disaster with a new shower head that won't leak? Would I need to fix the pipe itself as well?

I don't know if the shower are is bad or not, but those are standard threads in the wall. Any standard shower arm with shower head will fix that.
Or for a very simple fix, you might try unthreading just the shower head, and thread on a new one. Some will need tape on the threads, three wraps.

Just where it is leaking? In the wall, from the shower arm, at the arm to shower head joint, or is it coming out the side of the head itself?

As said, all you may need to do is to unscrew it, then add some pipe dope or PTFE tape on the threads, then tighten it back down. Some of those heads use a washer to seal, but most of the time, it's a standard tapered pipe thread which doesn't seal without either pipe dope or tape.

A new shower head will be flow restricted...your old one may not be.
