I have a StarBath rainfall shower system with a rainfall head coming out from the wall (not ceiling) and a hand shower attachment (no tub spout). The wall behind the shower system is currently open. Can I replace this with a new system without ripping out more tile from the inside shower side?

If the wall behind is open, how about a picture of the backside? But in theory, yes, you could replace everything from the backside and reuse the existing holes in the tile/waterproofing for your new shower valve/shower head/hand shower.

Depending on the hardware you choose, you might need to reshape one of the holes...the most common one is probably the main valve, but it really depends on what size the existing hole is now, and the valve and trim you choose for the new one. The valve can probably be made to be like new with a new cartridge or two...changing the rainshower head should be easy. If you needed to cover up the hole for the handheld, that might be a problem unless you found something you liked to cover it up or install in its place..
