Vent for shower toilet code compliance

Please see attached crude diagram, and tell me if existing meets code or if additional venting is required.

You can't wet vent between floors. Looks like you need to break concrete to add a vent.

Is it not acceptable to start a vent right where the 4” emerges from concrete? There would be 1-1.5’ of wet vent?

US codes state that vertical wet vents can only be used when all fixtures are on the same floor. Check with your local building/code/inspection department about what specific rules are adopted.

Depending on the rest of your plumbing there may be other options. Usually the bathroom sink's vent provides the wet vent for the shower and toilet.
