Moen Standard. 1225 cartridge. T470 for crystal handle, TL470 for metal lever handle. Moen customer service is excellent. Will most likely send free parts.

Moen conversion adapter from old style to moentrol 145058.
You have an old style Moen. Cartridge pulls in and out for on and off and left and right for temperature. the two screw holes that hold the plate on the wall are side by side under the handle.
On Moen's website you can filter only "Moentrol" shower trims and choose what you want. The conversion adapter relocates the screw holes from under the handle to what now is standard on moentrol valves (screw at 2 o'clock and 7-o'clock).
Be Aware some plates are concaved and you may have to enlarge the opening in the wall to allow the plate to sit flat.

Turn water off. Remove aftermarket universal handle adapter. Remove retaining clip. Swap cartridge with new 1225.
