changing cast iron flange

I have recently noticed that my cast iron pipe is broken away from the cast iron flange on my first floor bathroom. first of all I would like to know what would be the most likely reason. second; what would be the most likely way to fix the problem.

If you have a basement replace it with PVC. It may have rusted out, it is hard to say, I can't see it from here.
