Need help identifying Grohe (2008/2009 year) Pressure Balance Valve Trim

The sleeve is getting loose - any idea what to do in this case?

If I can find the model name, maybe a manual will help.

Appreciate any thoughts.

The handle wobble is most likely the handle adapter screw has come loose. you first need to remove the handle to get to the handle adapter. access the handle set screw by sliding the grooved rings (with much effort) towards the wall. once separated a small gap will reveal a setscrew at the 6 o-clock position. the Grohe book shows a 3mm allen wrench is used to unscrew the handle form the handle adapter. once the handle is removed, tighten the adapter and reinstall the handle.
in a perfect world this is an easy task. However the set screw might be stripped or corroded and needs to be drilled out. Your trim is discontinued so the chances of finding a replacement handle is slim.
worst case scenario is you can purchase any current grohe shower trim that uses the 3.0 rough. Grohe makes a conversion adapter to convert your old valve to the new style trim kits.

Thank you for finding the correct Grohe model!

What is wobbly is not the handle but the cap - item 3 in your diagram.

You were right that the set screw for the handle is stripped and I'm so far not able to remove it.

The question is, if i force off the handle, will I be able to secure the cap? I'm not sure what secures it - I remove the escutcheon (#2) and looked behind the trim, and am still not clear what the cap attaches to.

Any thoughts?

The chrome cap or sleeve friction fits over the o-ring on the valve body. Push this sleeve further into the wall by twisting and rocking it so it passes over the o-ring. see picture.

Question for TJRAD/Grohe please:
Are those integral shut off valves for hot and cold?
Is there a replacement part/cartridge that can be used so the faucet doesn't drip?
