So when we bought our house we knew the main stack was leaking. We discovered a hole right beneath the clean out and patched it up with some epoxy putty. I think I got a good seal around the hole, but now I think the cleanout itself is leaking as well.

My question is, how do you get a good seal with your cleanout fittings?

It depends on what you have. It is possible that some PTFE on a new plastic cover would do it. You might need to use an expansion plug that expands rubber to seal.

If your plug leaks often, you probably have a blockage also. Otherwise there would seldom be water to leak. If you have a septic tank, get that emptied now if you have not done it recently. If you have city sewer, or if the septic has been pumped recently enough, consider getting your drain lines rodded out.
