American Standard T675.500 trim & R121SS Shower valve

Hello,

I wanted to replace the hardware on one of the showers in a house I bought. I looked up the variations / types to make sure I bought the right one. After actually trying to do the replace it looks like I have some weird variation of American Standard that I can't find. At first I thought it was posi temp but someone installed it upside down.

(I see American Standard in writing but wasn't visible until I cleaned it)

Anyone know where you can actually find hardware like this? I only see American Standard that is 2 centered screw holes.

I want to replace the whole kit but I don't see any American Standard products with the same screw hole placement in the plate like this one. Usually they are side by side. This one is setup like Posi Temp flipped. Was hoping to do a straight hardware swap without replacing the valve.

The shower head is trivial but if you know a place I can source handles/plates of this type that would be awesome.
