Help identify Delta faucet

I need to replace the hose on this faucet, but I don't have the model number. Apparently there is more than one model number for this type of faucet. The faucet is over 10 years old.

Thanks, but like I said, there are several that look alike, but have different model numbers.

I emailed moen a picture of my faucet. They identified it and sent me new handles for free

Called Delta. They had my name on file. Send the part for free. 15 year old faucet!
