I had a broken pvc flange, so I used a 4" hole saw to try to cut out the old flange, but it turns out the elbow that the flange goes into is 4" outside diameter and 3.5" inside. That doesn't seem to be standard 3 or 4 inch. Any idea what I have?

Nevermind. I figured it out. Found a 3.5 inch PVC flange and used a 3.5 inch hole saw to get the old fitting out.

3 inch PVC is 3.5 inch OD.

A 3 inch spigot closet flange would be 3.5 inch OD.

So I now suspect you had a regular 3x4 inch flange that you cut away. Sorry I didn't think this through the first time you posted.
