Cleanout needs to be modified

I had a plumber out yesterday to fix a clog (he did a great job) and after he finished he recommended that I have the cleanout elevated a foot or so. The cleanout rises out of our slab at a shallow angle and the cap fitting is only an inch or so above the slab. If I graft on any extensions I imagine I will need to angle it straight up, or nearly straight up. Would a sleeve or coupler join the existing threaded cleanout fitting, or would I cut that off and glue in a coupler? Would the coupler fit around the outside of the old pipe, or the inside? The existing pipe is pretty close to the slab and as I mentioned, at an angle.
