Ultra-Mix, Can anyone help identify this American Standard faucet model?

Hi, thanks for help I am trying to figure out the parts for this faucet to replace the cartridge and pressure balance unit. I just move into this condo and both shower in condo have same faucet and no cold water coming out of them. Thanks so much.

So I got everything to work but got some leaking from the volume control shaft. Do you just pull it out and lube the O-Ring or is there a trick to get the shaft off?
