Does a 12 1/2” rough in mean buying a 12” or 14” round toilet?

Definitely not 14. There may be other options, depending on budget and need/desire for space. An elongated toilet might even be doable, depending.

Elongated would make it hard to get into shower. This is small bathroom. Will 12” be ok?

Yes. There will be a little more space behind the tank than ideal. A 12 inch toilet with a 12 inch rough will normally have 1/2 to 3/4 inch space between the tank and the wall. You would expect another 1/2 inch of gap with a 12.5 inch rough. When installing, you may be able to slide the toilet toward the wall a bit.

"Rough-in" is the measurement used from the face of the unfinished wall to the center of the closet flange (toilet bolts).
The bowl size or type is irrelevent to that measurement.

Are you asking about the bowl diameter or the actual rough-in needed for your toilet?

I guess both? I want to know what toilet size to get to clear wall. I measured from drywall to center of bolt. Right now there’s about a 1/2 to 3/4” gap bet wall and tank lid

What was your measurement? if it is 12 1/2" then the 12" rough toilet would be what you would want; as far as size (bowl diameter or shape) that would be up to you with regards to available room in your bathroom. If space is an issue then find a 12" rough toilet that has the front of the bowl as close to the wall as possible.

There are a few models of Elongated toilets that fit a 12" rough in but project the same distance from the wall to the tip of the bowl as a regular round bowl toilet.

The rough-in determines the bolts to the wall fit, not how far it projects into the room. So, you need to decide how far from the wall you can live with comfortably, as, if you do have 12.5” from the bolt center to the wall, any 12” toilet will fit. How far the front projects into the room depends on the design of the toilet, and that can vary quite a bit...not every elongated toilet will stick out further than a different round one...depends on the model and brand you choose.
