I need to relocate my toilet flange 5" to the left for spacing between a newly installed shower and vanity for the proper clearances.

The vertical riser from the closet elbow is a 4" PVC pipe and is 2' in length underground.

Can 2 45 DWV PVC elbows be used in the vertical riser pipe between the toilet flange and the 90 that is underground?

If the two 45s doesn't get you there, try rolling a 90 over 45 degrees at the bottom, then 45 up to the flange.
