Toilet flange question

Hello everyone, my floor tile just got put in, and I am ready to install the toilet. I am wondering a couple of things:

1) Do you think I need to put on an extender? The floor is not level so the toilet will have to be slightly shimmed. The left side of the flange is about 1/8" above the finished floor, and the right side is about 1/4" from the finished floor.

2) What type of shims should I use, and any thoughts on wax ring vs the rubber gaskets I have been seeing recently?

Drop the bowl down first and position the shims at the back.
Lift bowl, drop single wax on flange.
Drop bowl back down and install the washers and nuts.
Hook up water, fill tank and flush twice.
Retighten nuts and then caulk most of the way around the bowl except at the back and clear polyseamseal.
