Sewer transition, backflow & cleanout

I have a 4" cast sewer line and want to add a cleanout & backflow just outside where the line exits the house... about 3' underground. Can I remove a section and use PVC and, if so, what is the proper connector for the transition... a stainless steel clamp? I could never understand why a cleanout would be placed in the basement so that you would have to drain it there to get a snake into the line. My other thought was to put a riser in the basement and a 90 at the top to extend it outside, and forget the backflow, above the ground level... basement sill is about 2' above ground level. Any recommendations?
