Need help identifying a goosekneck faucet make and model

I've got a leaking goosekneck faucet and I don't know what brand or model it is. I replaced the o-ring on the piece coming out of the base but it still leaks when you push or pull on the goosekneck. Any advice on what parts to get and most importantly, what make and model this is.
