I'm remodeling a bathroom and based on the new layout, I really need to move this 2" sink drain over 8.5". I was thinking about doing an offset with a pair of 45 turns but the joist bay only gives me around 9" of clearance so I don't have the height for that. The elbow is a hard 90 at the base of the existing vertical. I do have good access via the basement below so I can cut that 90 out and start from scratch if needed. What would you recommend to move this 2" drain about 8.5" over?

You are supposed to use a long sweep to transition from vertical to horizontal. So solutions that cut out that existing 90 should be used. Putting in a 45 or a 22.5 into the horizontal plane could help you move the input over to the right.

I would get a long sweep and some pieces of pipe to play with to find something that matches your objective.

You can reduce down to 1.5 inch for a lavatory, if it helps.

Is it correct to cut that exiting 90 out.....put a 90 back in its place that's now horizontal......bring it over and then sweeping 90 for the horizontal to vertical transition?

That should be fine, but is there any advantage to that vs using a 45 to the new 90? The answer may indeed be yes, because the long 90 might extend too far toward the back wall, where what you describe lets the transition from vertical to horizontal be farther from the wall, and still clear the joist.
