Moen Chateau, Trying to identify a Moen bathtub faucet

Hi Terry I'm trying to identify this Moen bathtub faucet that was installed in 1976

I don't need the cartridge I'm looking to replace the square trim kit. The legend that I seen are round.

You need the "adapter plate" that changes to bolt openings from the 5-7 orientation to the 2-7 the new plates use. Then you can use any Monticello Moentrol trim you want.

From a customers home built in 1971.
The cartridge was original and was almost impossible to pull out. They had quit using the tub.

Here it is with the new 1225 cartridge installed. Thanks Master Plumber Mark for the box you sent me. I installed three of them in the last week.

And she likes the new shower trim much better.
New cartridge, new trim, it's new.
