Moen 4983, Help identifying brand of roman tub faucet (Solved)

I have searched the forums and I haven't found anything that matches my faucet. Do you have any idea what brand/model this is?

Info:
- House was built in 1986, but not sure if the faucet is original to the house
- The faucet appears to have been painted with the original finish being chrome
- Located in Ontario, Canada
