Leaking lav faucet

Hi, I have a faucet (see picture) which is dripping water. It is almost steady, 1-2 drops every couple of seconds. How would I go about fixing this?

You can replace the faucet, or you can identify the proper replacement parts to repair it. Often identifying the make and model of the existing unit is the hardest part.
