Kitchen sink vertical to horizontal transition.

Hello Everyone,
I have to make various corrections to my 2 inch kitchen drain line. I have no clue how it passed inspection when my kitchen was remodeled 17 years ago. Anyway, since I have to replace a sanitary tee with a combo wye(horizontal to horizontal connection), I was wondering what other problems exist. For one there is a mild reverse pitch where the lateral is split by a coupling. So I have to fix that too. My question concerns the vertical that comes down from the sink and connects to a mid sweep elbow. I assume this should be a long sweep elbow. Is there supposed to be a cleanout here? My space is quite limited due to large gas piping. I will have to be very careful to ensure proper pitch. I drew a few diagrams attached to this post. So here's are my questions:

1. Do I need a cleanout?
2. If I do, are both images 2 and 3 acceptable?

I can work a long sweep in, maybe even the wye. Image 2 might be more challenging since the pipe is fairly tight to the joist.

1. Not sure. Is this fed via a trap adapter under the kitchen sink. Don't know if NJ accepts that as place to snake.

2.Yes, both acceptable. The thing you labeled as a wye is called a combo, which is a combo of a 45 and a wye.
