Note that IPC does not stand for Illinois Plumbing Code but rather International Plumbing Code. I think Illinois code is based on UPC but with modifications, and the code varies a lot in the state. U stands for Uniform, and it is generally more restrictive than the IPC is. The IPC would be closer to plain English, but even that probably does not meet what you would call plain English. The IPC is not relevant to Illinois.

For clarity, IPC was my own abbreviation. I'm referring to the Illinois Plumbing Code.

Here's more specific questions -
I have underslab plumbing in the basement which has 2-4" stacks going to the main floor, a 3" vent stack (which vents the toilet as well) and 3-2" drains for sinks and shower.
Does each require a cleanout? Can the cleanout be a san-tee or does it need to be a wye?

The common requirement, and it does require a bit of common sense, is that any pipe going through the floor requires a cleanout above the floor. In addition. cleanouts are required at any fixture more than 6' from the drain line connection. The "common sense" thing, is that you install them wherever they are needed, regardless of whether they are required or not. FEW cleanouts, above the floor, are "Y"s, most are either "threaded inlet test tees", or sanitary tees with an adapter and cap or plug.
