Help with Kohler tub faucet.

I need help identifying this tub faucet and also how to remove the knobs. The knobs have square ceramic insert that I can't seem to get out to remove them.

Look around and behind the handles for holding screws. I see that the escutcheons have tiny screws at their base.
If not, see if the handle cover can be removed with a utility knife and if yes, there must be retaining screws hidden there.

They are Kohler handles and they just "pop off" using a thin knife blade.
