Help replacing 2 pipe packer well adapter

Hey there, I am trying to repair a two-pipe packer well casing adapter before just giving up and digging a new well. Attached is a photo of what was there before. I can't find this part or manufacturer anywhere on the internet. What I have coming out of the floor is a 1 1/4" ID threaded steel pipe and it is coming out of a 2% pipe which is probably less than an inch off the floor. The other well casing adapters I'm seeing on the internet seem to be threaded and this one just has the 1 1/4" pipe passing through a bunch of rubber grommets. Any help/suggestions would be greatly appreciated.

This is awkward, but...
