Fittings to use in branch with two small vertical drops

Old home with many challenges. I am building a kitchen island that will have AAV. Question is how to tie the drain to the main. There will be two drops and I would like to have a cleanout somewhere. Also long sweep 90's ok to go vert to horizontal? The first drop is under the island in the basement, the next is another smaller drop, maybe 5".

I was thinking of using a wye with. 45 under the sink and putting a cleanout at the end. The photo is of the second horizontal run. The pipe sticking out of the wall goes into the main. I am also upgraded the branch from 1.5 to 2" pvc.

The transition from vertical to horizontal, at the lower left of your green line, will have to be a long sweep.

Thanks, no problem there. What about under the sink, Should I have a wye and 45 with a threaded cleanout? I also bought a combo 1/8 wye that I was planning on returning.
 
Here is the fitting I was going to use for the horizontal cleanout. Would be on the new diagram where it says cleanup.

I think maybe a 45 and a wye is supposed to be there instead but idk.

Basically something like this? Would this be acceptable in the basement under the kitchen sink?
