Farmhouse plumbing blues

Hello all. I bought a farmhouse that was built in the 80’s. It is in the Texas hill country and is on my private well. I have a softener.
in the last 60 days I have had two leaks. Please see attached photos. The leak is coming from where the yellow arrow is pointing.
It is coming up through the black slab sleeve. One leak is on an outer wall and the other is in the kitchen. I put water weld on the kitchen
leak and it has held until tonight. I’m fairly familiar with plumbing but this set up has me baffled. I had a plumber come 3 days ago to take
a look. He didnt Know what to do and said he would come back next week. I have a 300 gallon holding tank and a regular softener.
Does anyone have any guesses what I might be looking at? I thought maybe the Pex line was softened water? I do have to turn off the well
as well as shut off the softener in order for it to be off comepletly. Any advice on what you would do if it were your home? You guys are always
the best.

no magic bullets, sorry.
Expose and repipe repair as needed or rerun above slab. Something about water quality may have caused early failure.
