Does this plumbing layout look OK?

hi, just hoping to run this past a plumber to catch anything that's wrong or could be improved before implementing. This is for an addition/renovation of my house. Currently, the 3" and 2" pipes at the bottom are sticking up out of the slab and serve a half bath. All plumbing above that has yet to be done. As drawn, the sink for the downstairs half-bath is draining into the 2" pipe which also vents the downstairs toilet. I think upstairs bath layout is pretty clear. Only thing to note is that the part of the wall with the tub will be furred out about 4" since the room is >5' wide (that's why there's space to get the vent in the wall behind it). The framing is odd in some places because of the way the house was originally framed plus the addition. Also, the HVAC trunk duct is pictured, but not the various takeoffs & supplies, so there are a few additional obstacles. I'm in Virginia which I believe is IPC.

I really appreciate anyone taking a look at this,

Looks good to me under IPC. Tub is vented with a santee on its back. The trap side is on the side that would have been the top of the santee for a lavatory drain.

Thank you! Would you add any cleanouts? And if so, where?

I don't know about cleanouts.
Do make sure that 90, that goes from vertical to horizontal, under the toilet is a long sweep.
