Do I need put cleanout on secondary suite drain

I connect drain of secondary suite with five fixtures to building drain. I check the local plumbing code about cleanout fitting as bellow.

My connection to main drain is Y fitting. DO I need put a leanout fitting for the whole drain of secondary suite?
If yes, put cleanout on A or B point?

The connection is horizontal to horizontal . Is it a stack?

Good plumbing practice requires a clean out every change of direction greater than 45 DEG

At the base of each stack soil ,waste or leader and located every 50 feet (depending on pipe diameter)
