Disassembling an old shower faucet

I have a house in South Florida that was built in the mid to late 1990s. I have a bathroom that I suspect was remodeled at some point with a shower faucet that I have never seen before and cannot find online. The water does not get very hot and the tub diverter is impossible to pull out. I would like to try changing out the shower valve and diverter stem but cannot figure out how to disassemble this handle to get to the parts. Does anyone know what type of shower faucet this is and/or how to take it apart (photo attached)?
