Non-Philadelphia PA uses IPC (without amendments) rather than UPC. Philadelphia uses IPC with amendments.
1. Add a TTR vent or AAV to the standpipe trap.
2. Wye in the standpipe drainage downstream of the lavatory.
3. That wet vent from the lavatory needs to be 2 inch.

The toilet is unvented in your drawing. Washing machine needs to be downstream of a wet vent.
