Danze Shower handle with no set screw, no cap, no brand name

I am trying to remove the shower handle with no set screw, no cap or brand name. The shower head shakes constantly with hot water, and I suspect that the cartridge needs to be replaced. Can't figure out how to remove the handle so that I can get to the cartridge inside. It is in a high rise condo building built by Bosa 10 years ago in Vancouver area. The shower handle may be a European (Italian?) brand. I tried to pull, twist and do every other way to remove it, but it won't budge. Any ideas on how to remove this handle?

There is a small set screw behind the lever that allows you to get to the main handle screw in front. See page 9
