Cleanout confusion

I have read the UPC cleanout requirements and am now thoroughly confused!

So bottom line me...is a cleanout required for an main floor lavatory sink?


If a cleanout is needed, do you position it above the san-tee or below?
I have read arguments for each: above allows you to run water through the trap while rodding the line.
Below is easier to access since the sink bowl and water lines won't be in the way.

If the lav is within five feet of a line that has a cleanout, you're fine.
A cleanout can be above or below.

On a different topic, do you have a recommendation for vent pipe flashing for a flat roof (1.5 in 12 pitch)? So far, I am only finding boots for pitched roofs with shingles...
