Adding floor drain w/ clean out?

I'm adding a floor drain to an existing, buried sewer ejector. This addition involves digging up an area next to the ejector's 4" inlet and splicing in a wye on its side that branches off a 2" for the floor drain. The vent for the floor drain is a tee on it's back that goes to an AAV (my only option).

My question, do I need a clean out for the floor drain? If so, is the one I've pictured below ok? If it matters, this addition is a part of a remodel for which I've pulled permits for. That said, it will be inspected.
