    That bathroom can be a single horizontal wet vented system too. Up and over to the toilet, branch off the horizontal for the shower, then another branch to the sink and up through the roof with the vent. Done. About 10 fittings in the bathroom.

    I think that is likely the valve opening that creates the flush. Again, not familiar with that brand but the Neorest that its modeled after has some mechanical valves that open to create the flush and they do make some noise as the pressure is released.

    It doesn't look properly sealed to me. A toilet flange should be flat. The black plastic looks like something someone placed inside of a 4" flange and then stuck the plug inside of that. If it wiggles or moves at all then its not sealed. I've never used this product, but looks like it should...
