Looking for waiver if liability

Just went out on my own doing a sewer, drain cleaning, and service plumbing repair business. I have been using generic invoives from office depot, but i need to make proper invoices that have and have a waiver of liability for things such as problems arising from faulty plumbing, rotten drain lines, corroded pipes, cable getting stuck in broken sewer line etc.. and have the customer sign it prior to doing work.
Can some of you share a good waiver statement that you are using with me?

In America most civilized states the waiver means nothing . You can try to have them sign a paper that your not responsible but if your taken to court most likely your insurance company will settle the claim before it goes to a judge or jury
most importantly then a waver have great coverage .

We completed a sprinkler job on the 86 floor on a building in Manhattan and I took a 20 MILLION dollar rider out just in case there was a leak
