Looking for a replacement part for this pitless adapter.
The lever / cog is cracked.
Can anyone help me identify the brand/model ?
And tell me if I can just replace the lever?

I dont...i have seen a few but don't know the brand name....they have been mentioned in some earlier posts here you might be able to search and find them. A good welder or machine shop could probably fix that if you can't get a replacement part quickly...or you can dig up and replace whole thing if it's real water emergency.

I had suggested in his first thread that if this pitless is a clearway/throughway pitless, he could probably use the in-the-casing part of a Merrill MCK/SMCK pitless and keep his existing outside-the-casing connection. https://terrylove.com/forums/index.php?threads/replacement-part-for-pittless-adapter.94563/

A family of cast brass well casing saddles designed to clamp flush to the
exterior surface of 4, 4 1/2, 5, 5 5/8 and 6 inch diameter steel and PVC
well casings. A solid Buna-N 'O' ring is used to seal the saddle against the
outer casing surface to seal the well casing water-tight. The cast brass
riser pipe elbow is designed to screw/wedge pressure tight against the
inner wall of the well casing to provide positive system pressurization of
the point of outer saddle connection to the well casing. To accommodate
this design feature, a riser pipe extension is designed to be supported by a
cast 'T' on the top rim of the well casing. Zinc plated steel 'U' bolts are
provided with the adapters for use on steel well casings.

That sure looks like it - thanks! I didn't get a picture of the other size of the adapter.
It's up at the cottage and we won't be up there again for a few more weeks.

BTW, if you google "Patent 3430697", you'll get a hit for a pitless adapter patented by William Wellstein. Looks exactly like yours
