toilet flange install on tiled floor

I am doing a bathroom remodel and am currently planning out the installation of the toilet flange in relation to the tiled floor. I am finding conflicting information of the appropriate way to proceed. Some threads indicate that the flange should be attached to and lay on top of the finished tile floor. Others indicate it may lay on the subfloor and then tiled around. The prior (finished floor) seems the most logical to me and would provide the best watertight seal with the wax ring. In some threads the later (subfloor) may require two wax rings and that to me sounds suspect. What is the best way to proceed?

If I am to install the flange on the finished floor, what is the best way to drill the holes for the flange? My floor consists of �� plywood, �� backerboard and then �� travertine tile. Would a masonry bit be appropriate or should I take the piece to a tile shop?
