I do it with the water on, but since you haven't done it before, i would recommend you turn your water off first. you may also need to remove the handle to get the packing nut back far enough.
If you do it with the water on, do not open the faucet until you have got the packing nut started a couple turns. The only thing holding the stem assembly in the faucet is the worm gears and the packing nut.
