No, it isn't normal for a wax ring to fail, but that's assuming all installation details were done correctly, including the flange resting on top of the finished floor. It sounds like the tile was laid around the flange so that the new floor level is above the flange probably 1/4" or so. That's why the longer bolts were needed. This space can be compensated for with the wax ring, but perhaps when the toilet was reset, the new ring wasn't thick enough. The only way to know for sure is to pull the toilet and find out.
