Toilet Flange Installation - New Construction

Toilet flanges on our second floor were directly set on top of the wood subfloor. Our finished floor height will be approximately 1 3/4" to 2 inches including a 1 1/4" light concrete underlayment. Comparatively the toilet flange on our first floor is raised above the slab on grade. Please confirm the appropriate method for installing the toilet flange on second level wood subfloor. Given the aforentioned field conditions should it be raised to accomodate for the finished floor or would it be better to form around the flange to pour the concrete underlayment? Thank you.
