The flange is usually installed when the toilet will be set on the finished floor.
If in fact you have a lead bend, you would need to chip around it very carfully, not to damage the lead. Chip away only enough to install the closet flange (for lead pipe). Slip the flange over the lead so the flange sits flush on the floor. Cut off the excess lead leaving only enough to bend over the flange, up to the point where the closet bolts slide in. Anchor the flange to the floor and that should be it. Some plumbers will solder the lead to the brass closet flange but this not necessary.
