The flange has to be solidly attached to the drain. Since the flange is attached to PVC,I wonder if the the flange is cast. Most likely it is PVC with a metal ring that should be bolted to the floor and that the toilet is attached to. If the rust has degraded the metal, I'd look to replace the flange. The problem is the concrete. It will have to be broken out enough to gain access to the drain line below so that you can cut off and replace the old fitting. Keep in mind the final result must have the flange setting level on top of the finished floor and anchored into the concrete. It will be very difficult to get everything to fit exactly right. I think you will be time and money ahead to have a plumber handle this.
