Hot water in the cold water faucet.

I just came back from a friends house that has an odd problem. When she first turns on the cold water, hot water comes out, then after a little bit, cold water comes out. And I mean really hot water too.

It's a gas water heater. She says her bills are astronomical. How's $300+ in gas bills for one month. I put my hand on the T&P drain pipe and it was as hot as the hot water outlet. But, doesn't that just drain out somewhere?

Also, when I put my hand on the cold water inlet, it was nice and cold. The arrow on the one way valve was pointed in the right direction as well.

Since the three pipes, cold, hot, and T&P disappear into the wall, I have no way of knowing what's going on behind the scenes without ripping apart some sheet rock. I'd like to know if anyone knows how this might be happening before attempting any major changes to the system, but with such high bills, it will be cheaper to rip things apart sooner rather than later.
