The top of the drain pipe is flush with the floor. The elbow is 4" O.D. With is exactly the size of the 3" toilet flanges. So I can't even slip one into the pipe if it was the correct diameter.

Also the I.D. of the 4" toilet flanges are 4.5" obviously leaving a 0.5" gap if I were to try to slip the flange over the elbow.

Am I missing something here? Is there a flange to even fit the current elbow that is on this drain?

: I think you'll have to go under the floor and cut the pipe back so new fitting can be installed. Problem is, there must be a 1/4" per foot slope in the drain, and since the closet bend is flush with the floor, this could mean the drain pipe is setting so high the new fitting will not allow the slope. If that's the case, you will have to find a way to lower the drain pipe to get the slope.
