1920's American Standard Shower Valve Won't Come Offf

I have a shower stall in our 1926 home where both the hot and cold valves leak. I tried to take of the hardware to rebuild it, but the handles won't come off. I've removed the index button (little 'H' sign) and the handle screw. At that point it looks like the handle should come off, but it won't. They're porcelin 'X' shaped handles, and they appear to be molded around the valve stem. What am I doing wrong?
