There are degrees of the first two questions. A slight angle will not present too much problem, but a some point it would. What's level, and are you talking level as sloping or level as in smooth. Of course, a floor should be level, but it it is true to itself, it wouldn't really hurt if it a a slight slope. If you're talking smooth, then smooth it out. There are leveling products to do it with. Personally, I like things smooth even when out of sight. The 10" rough in isn't a problem. Nibco makes a flange that gives 2" plus or minus adjustmnet, so you can use a 12" toilet. Some toilets are made for 10" rough in.
