If the drain is a 4" ABS pipe, you can get a flange that will fit inside, but you must shorten the pipe below the floor level. If you have a 3" ABS pipe, then the flange fits over the pipe just like any other fitting. The depth of the flange is 2-1/2 below the floor level, so you will have to chisel out that deep and cut the pipe 1-1/4" below the floor level. I just did that and posted on how I did it. A real plumber posted about an inside cutter that he uses. The space around the pipe doesn't need to be too wide except getting a chisel in deep enough requires a bit more room than the fitting. Mine was fresh concrete and I hadn't allowed enough depth so I had to enlarge the space some. New concrete is easy, old is hard (literally)
