To my knowledge, there is no flange that will fit inside a 3". I think you're stuck with chiseling. It will take a 3" deep channel for the flange to slide on to the pipe. This shouldn't be too bad of a job, a rotary hammer will make fairly quick work of breaking out the concrete. Just don't get too close to the pipe and break it off! When you fill and patch the hole, you will want to smooth the concrete pretty well then use floor leveling material to smooth it out. It really won't have to be absolutely perfect as the toilet will cover. Just don't leave it rough or uneven.
