low pressure on hot water side

My home has an A.O. Smith water heater. The house was built new 6 years ago. Approximately 1 month ago we began to experience very low water pressure on the hot water side only and a reduction in the amount of hot water available. I have tried cleaning filter screens, but it helps little. When I do clean these screens I keep getting lots of small peices of calcium deposits. We are on city water and have no other water filtration system. What could be causing my hot water problems?
